Job Description:

We are seeking a compassionate and experienced Registered Nurse (RN) to join our team. As an integral member of our healthcare team, the Registered Nurse will be responsible for providing exceptional patient care, collaborating with physicians and other healthcare professionals, and promoting a positive patient experience. Looking for a experienced Home Care Nurse that has knowledge of intake assessments, insurance guidlines, and all the in's and outs of Home Care.

Responsibilities:

• Assess patients' health status and develop individualized care plans in collaboration with the healthcare team.
• Administer medications and treatments as prescribed by physicians and monitor patients' responses.
• Perform comprehensive nursing assessments and provide skilled nursing care to patients in various settings, including hospitals, clinics, and community health centers.
• Educate patients and their families about their medical conditions, treatment options, and self-care techniques.
• Coordinate patient care activities, including scheduling appointments, arranging referrals, and communicating with other healthcare providers.
• Document patients' medical histories, assessments, and treatment plans accurately and efficiently.
• Adhere to nursing standards and protocols to ensure the highest quality of patient care and safety.
• Participate in quality improvement initiatives and professional development activities to enhance nursing practice and patient outcomes.

Qualifications:

• Valid Registered Nurse (RN) license in the state of [State].
• Bachelor of Science in Nursing (BSN) preferred but not required.
• BLS certification; ACLS and other relevant certifications may be required depending on the specialty area.
• Excellent clinical assessment and critical thinking skills.
• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment and collaborate with multidisciplinary teams.
• Commitment to providing patient-centered care and promoting positive patient outcomes.
• Must have atleast 2 years of home care eperince and 1 year of supervisory experience in home care.

Job Types: Full-time, Part-time

Pay: $35.00 - $45.00 per hour

Expected hours: 40 – 50 per week
